We're here for YOU

Our Mission

Our mission is to promote health, independence, dignity, and quality of life for older and younger adults facing serious health challenges by providing active health care and social support to uplift them and their families in a spirit of friendship and community.

about Our Program

What is Adult Day Health Care?
Participants come to our center during the day to work on their health and wellness goals and enjoy friendship and connection. Unique in Butte County, we are able offer care and support  in an uplifting community setting. Combined with caregiver support, our comprehensive program can help the whole family thrive.

Person-Centered Care
We work with you and your family to create a program of services tailored to your needs and interests.

Our Comprehensive Approach
Our team includes social workers, nurses, rehabilitation therapists, and other health care professionals dedicated to supporting you and your family.

Who do we serve?
We serve adults ages 18 to over 100, with conditions such as strokes, diabetes, respiratory or cardiac illnesses, dementia, and other diagnoses. We offer a personalized treatment plan developed by our team of health care professionals. Services are available between one and five days per week, depending on your need.

What services do we provide?
We offer personalized care based on your needs. These can include social work services, nursing and personal care, health monitoring and education, medication management, nutrition counseling, physical, occupational and speech therapies, and specialized support for participants with memory loss.

What are the benefits?
Adult Day Health Care takes place in an enjoyable, clubhouse atmosphere, where participants are able to connect with friends, pursue hobbies and special interests, have a nutritious, freshly-cooked lunch, and more, all while continuing to live at home or with their families. When participants are able to see friends and partake in fun activities while also having their health needs met, and caregivers are able to have respite during the day, we see the whole family thrive.

What is the cost?
Your services may be covered by Medi-Cal, local fee waiver programs, or your long-term care insurance. If paying privately, fees are on a sliding fee schedule, based on your ability to pay.
